HIPAA Notice of Privacy Practices

This notice describes how I Can Grow ABA Behavior Therapy may use and disclose protected health information (PHI) and how you can access your information. Please review it carefully.

1. Your Protected Health Information

PHI includes any personal health information related to your child’s diagnosis, treatment, or insurance that identifies them. This information is protected under the Health Insurance Portability and Accountability Act (HIPAA).

We may use and share your PHI for the following purposes:

  • Treatment: Sharing information with therapists, BCBAs, or healthcare providers for care coordination.

  • Payment: Submitting information to insurance companies for billing or authorization.

  • Healthcare Operations: Quality improvement, audits, or staff training.

  • When Required by Law: To comply with subpoenas, court orders, or mandatory reporting.

3. Your Rights Under HIPAA

You have the right to:

  • Access and request a copy of your child’s PHI

  • Request corrections to inaccurate records

  • Receive a list of disclosures (accounting of PHI sharing)

  • Request limitations on who may access your information

  • Request confidential communication (e.g., via mail or private email)

  • File a complaint if you believe your rights have been violated
